Smothermon Soccer L.L.C. is providing summer soccer camps for beginners, intermediate players, advanced players, and teams. The camp instructors will be Louie Smothermon (USSF “A” License, NSCAA Premier Diploma, USSF “Y” license, CSA Director of Coaching, former New Orleans Storm Assistant Coach/player), Winston Lewis (USSF “B” License, USSF “Y” License, CSA staff coach), St. Nick Benjamin (Lindsay Wilson University Assistant coach), Lester Young (CONCACAF/UEFA Coaching Course, Olympic Solidarity Coaching Course, former T&T U17 National team Assistant Coach) and other professional, college, high school coaches or players. The camp dates are:

June 26-30 CSA fields in Audubon Park (beginner, intermediate, advanced; half or full day option 9am-12pm $95, 1pm-4pm $95, 9am-4pm $155)

June 26-30 PAC fields- Westbank (beginner, intermediate, advanced; half or full day option 9am-12pm $95, 1pm-4pm $95, 9am-4pm $155)

July 10-14 Brother Martin H.S. Soccer Camp (for beginners, intermediate, advanced players ages 5-14; half day only 8:30-11:30am $85)

July 10-14 Brother Martin Crusader training camp (for current Brother Martin Students at the CSA fields in Audubon Park 5:30-7:00pm (8^th and 9^th grade, 7:00-8:30pm $75)

July 17-21 J.B Martin Fields in St. Charles (beginner, intermediate,

advanced, teams; half day 9am-12pm $95 and full day 9am-4pm $155)

July 24-28 CSA fields in Audubon Park (beginner, intermediate, advanced; half or full day option 9am-12pm $95, 1pm-4pm $95, 9am-4pm $155)

July 24-28 PAC fields- Westbank (beginner, intermediate, advanced; half or full day option 9am-12pm $95, 1pm-4pm $95, 9am-4pm $155)

July 31-August 4 CYSA fields in Covington (beginner, intermediate, advanced, teams; half day 9am-12pm $95 and full day 9am-4pm $155)

August 7-11 Carrollton Boosters (beginners, intermediate players)

*Half day only 8:30-11:30am, $85

August 7-11 Rec Plus and Metro evening team camp (Metro 5-6:30pm; Rec Plus 6:30-8:00pm M-F)- * teams sign up on an individual basis $50 per player-minimum of 10 players

August 14-18 CSA Premier team camp-*CSA Premier players are required to attend

*The summer soccer camp focus is to provide fun, age appropriate, professional training. The camps are designed to meet the needs of every player at every level.

Permission to participate, Release of Liability, and Authoriztion for Medical Treatment. Smothermon Soccer Summer Camps June 1 - August 30, 2006

The named participant is in good health and has my permission to participate at the Smothermon Soccer Soccer Camp. I hereby release Louie Smothermon, the Carrollton Soccer Association and all of their respective employees, officers, directors, agents, officials, and volunteers from and against any liability claims and demands for any injury or illness incurred at the Smothermon Soccer Soccer Camp. I hereby assume complete financial responsibility for any personal injury or property damage created as a result of an intentional or negligent act of my child or ward while he or she is attending the Smothermon Soccer Soccer Camp. If my child needs medical treatment and my consent cannot be obtained, I hereby agree that Louie Smothermon or other Smothermon Soccer Camp personnel may consent to appropriate medical treatment for my child. This release will be in effect during the camp dates shown above. I will be responsible for any costs of medical treatment incurred during this period of time.
