Ages: 9-12 years old (8 years old if they play up) Dates: June 6^th – June 10^th

June 13^th – June 17^th (Makeup Dates) June 20^th – 24^th

School Days: Monday – Friday

Time: 8:45a.m. – 11:45a.m.

(June 6^th- Registration starts at 8:00 a.m.)

Location: Plantation Athletic Club (PAC Field – Westbank)

Cost: $300

($150 Non-Refundable Deposit due by May 30^th to reserve spot)

($150 Balance due June 6^th upon registration)

($375 is due for walk-ups or late payments)

(Note: There will be a minimum and cap on the number of players allowed in program)

· All players will be supervised by instructors from the time they are dropped off until departure. Please arrange the proper transportation for your player to be picked up by 12:00p.m. Also, there will be an athletic trainer on site for any serious injury that may occur. All players are covered thru By-You Gurus Baseball, LLC insurance policy.

What to Bring? Glove, Cleats, Bat, Batting Gloves, Hat, Shorts, Towel, Sunscreen, Catchers (Gear), Helmet, etc.

Please mail registration sheet, liability / ground rules form (can be printed off the web @ www.by-yougurus.com), and include cash or check (made out to Luke Fortenberry) by May 30^th, 2005 to reserve your spot!!!
